The account application is required for all accounts. This simple form gathers all required business information to get your company setup.
The credit application is only required if you are applying for an open account with us. If you are a credit card or prepay customer, you do not need to fill this out.
The Kentucky Resale Certificate form is only required if you are sales tax exempt. If you are exempt, we must have this form on file before we can remove sales tax from your invoices.
